Creating a safe environment and teaching important life skills is core to Scouting.   We teach our Scouts to BE PREPARED!

Part of being prepared is having access to necessary medical information in case a medical emergency occurs.  Having current and accurate information about allergies, health history, and medications can mean the difference between life and death.

Every Scout, Adult and Siblings that participate in Pack activities must complete and submit the Annual Health and Medical Record forms to Pack Leadership per BSA policy.  The Forms can be accessed from the BSA Website.

Complete Form A & B1 and B2 and submit the completed forms to the Cubmaster PRIOR to participating in any outings.  

If your Scout participates in any activities lasting more than 72 hours and/or Day Camp they will also be required to submit Form C:  Pre-Participation Physical which requires completion by a medical professional.  The Counci has, in the past, hosted days where Cub Scouts could obtain free physicals for sports and/or camp.
